Good Documentation Practices Dos and Donts (GDP):

Good Documentation Practices (GDP) Do’s:

  1. Use the defined colored ball pen for document entry.
  2. Use the defined date and time format.
  3. Make document entries in a legible way.
  4. Make contemporaneous document entries.
  5. Make entries to all the applicable cells. If not applicable put —NA—
  6. Make entry directly onto the authentic format.
  7. In case of multiple options to select from; encircle the applicable option.
  8. Identify each entry with signature, date and time of entry.

Good Documentation Practices (GDP) Dont’s:

  1. Do not sign for anyone else.
  2. Do not make document entries in advance.
  3. Do not make document entries in back-date.
  4. Do not make document entries for compliance (Falsification).
  5. Do not make document entries on unauthorized forms.
